Siv Hun, Master of Social Work (Qualifying)

Siv is a qualified Social Worker with Footprints Community, currently delivering services at Smith Brothers Medical Clinic through Brisbane South PHN’s Social Worker in General Practice project. Siv is also an accredited Mental Health and Youth Mental Health First Aider, with experience supporting individuals with complex psychosocial, cognitive and physical needs across community and healthcare settings.

 

She has worked extensively within the NDIS framework and is passionate about delivering trauma-informed, person-centred care and works collaboratively with GPs and multidisciplinary teams; providing compressive psychosocial and needs assessments, and coordinating services for individuals experiencing mental health challenges, housing instability and social disadvantages. Siv has also supported individuals through complex life situations, navigating legal systems and barriers to accessing healthcare and social supports.

 

Siv is multilingual and values culturally responsive practice, ensuring patients feel heard, respected and supported in their healthcare journey.
